AIHA is an autoimmune disorder characterised by the premature destruction of healthy red blood cells (RBCs) by auto-antibodies. It is a common disorder directed against self RBCs which can be idiopathic (primary) or secondary. In w-AIHA, the autoantibodies react optimally with the RBCs at 37C. Normally, red blood cells live in the body for 100 to 120 days. However, in severe cases of AIHA, the cells may remain only for a few days. In children, it is a rare condition that is usually temporary. In some adults, however, AIHA can be a long-term condition that returns frequently. AIHA can develop rapidly or over time.
AIHA is a decompensated acquired haemolysis caused by the host's immune system acting against its own red cell antigens. The incidence is 1 per 100,000/year and approximately half of the cases are secondary to an associated disorder. Serologically, cases are divided into warm, cold (cold haemagglutinin disease and paroxysmal cold haemoglobinuria) or mixed AIHA.
